Conditions

All 15 conditions and what they do

Blinded & Deafened

Blinded
Can't see; automatically fails sight-based checks; attack rolls against you have advantage; your attacks have disadvantage.
Deafened
Can't hear; automatically fails hearing-based checks.

Charmed & Frightened

Charmed
Can't attack the charmer or target it with harmful effects; the charmer has advantage on social checks against you.
Frightened
Disadvantage on ability checks and attack rolls while the source is in line of sight; can't willingly move closer to the source.

Grappled & Restrained

Grappled
Speed becomes 0. Ends if the grappler is incapacitated, or if you're moved out of the grappler's reach by an effect.
Restrained
Speed 0 and can't benefit from bonuses to speed; attack rolls against you have advantage, yours have disadvantage; disadvantage on Dexterity saves.

Incapacitated, Stunned, Paralyzed, Unconscious, Petrified

Incapacitated
Can't take actions or reactions.
Stunned
Incapacitated, can't move, speaks falteringly; attacks against you have advantage; auto-fails Strength and Dexterity saves.
Paralyzed
As stunned, plus any attack within 5 feet that hits is a critical hit.
Unconscious
As paralyzed, plus you drop what you're holding and fall prone; attacks within 5 feet auto-crit.
Petrified
Turned to stone with your weight ×10; incapacitated, can't move or speak, unaware of surroundings; resistant to all damage; immune to poison and disease.

Invisible, Poisoned, Prone

Invisible
Impossible to see without magic or special senses; attacks against you have disadvantage, yours have advantage.
Poisoned
Disadvantage on attack rolls and ability checks.
Prone
Your only movement is crawling (unless you stand: costs half your speed); disadvantage on attacks; attacks against you have advantage within 5 feet, disadvantage beyond.

Exhaustion

Six levels, each worse than the last: 1) disadvantage on ability checks; 2) speed halved; 3) disadvantage on attacks and saves; 4) hit point maximum halved; 5) speed 0; 6) death. A long rest removes one level (with food and drink).
